Job Description:

communities. We seek a dedicated Stroke Nurse Navigator to join our
team, a role pivotal in guiding patients through the process of
recovery from stroke. This position involves working closely with a
multi-disciplinary team to ensure seamless care and support for
patients from diagnosis through treatment and beyond. The ideal
candidate will have a deep understanding of neuroscience nursing
and be skilled in patient education, case management, and
coordination of stroke care services. Provide education and support
to patients and their families regarding stroke prevention,
recovery, and lifestyle modification. Coordinate multidisciplinary
team efforts to tailor individual care plans for stroke patients.
Assist in the management of patient care from initial diagnosis to
recovery, including follow-up appointments and rehabilitation
services. Monitor patient progress and adapt care plans as
necessary in consultation with healthcare professionals. Facilitate
patient access to necessary medical and social services within the
community. Participate in the development and implementation of
patient and staff stroke education programs. Track outcomes and
provide feedback to improve the stroke care program. In-depth
knowledge of stroke pathophysiology and management Strong
organizational and communication skills Ability to work effectively
within a multidisciplinary team Proficient in patient education and
counseling Experience with healthcare information systems and
electronic health records Medical Insurance Dental Insurance Vision
Insurance Life Insurance Current Registered Nurse (RN) license
Certified Neuroscience Registered Nurse (CNRN) preferred Stroke
Certified Registered Nurse (SCRN) certification highly
desirable
