Job Description:
Agape Care Group is looking for a compassionate and dedicated
Hospice Chaplain to join our team, providing spiritual and
emotional support to patients and their families during the end
stages of life. Our mission is to offer comfort and dignity through
tailored care plans that address the spiritual needs of each
individual. As a Hospice Chaplain at Agape Care Group, you will
play a pivotal role in our interdisciplinary team, using your
spiritual insight and pastoral skills to enhance the quality of
life for those in our care. Provide spiritual support and counsel
to patients and their families. Perform spiritual assessments and
create plans to meet the needs of patients. Conduct or arrange
religious services according to patient and family preferences.
Liaise with local clergy to arrange community support as needed.
Document patient care efficiently in their health records.
Participate in interdisciplinary team meetings to discuss and plan
patient care. Offer grief counseling and bereavement support
services for families. Excellent interpersonal and communication
skills. Strong ability to provide spiritual guidance and pastoral
care. Capability to work sensitively and effectively with diverse
populations. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Ordination or endorsement from a recognized
religious congregation. Certification with the Association of
Professional Chaplains, Board Certified Chaplain (BCC)
preferred.
