Key Responsibilities Collaborate to provide management and delivery of patient care plans Support patients in Lewisburg and surrounding areas Meet critical deadlines for charting Self-manage time and schedule Drive to/from patient locations Schedule and Shift Details Part-time Sat & Sun 8am-8pm Travel Local travel to (Counties) Some of Lycoming & Snyder; parts of Union, Northumberland, and Upper Dauphin. Also growing in Schuylkill, Clinton, and Centre Top Skills Required 1 year of professional experience as an LPN (Home Health experience preferred) Active LPN License in the State of practice Associates Degree (or higher) in Nursing BLS and CPR Certification (if you don't have this - we will help you get it!) Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance Hospice LPN, Weekends Part-time City/State: Lewisburg, PA Territory/Driving Expectation: (Counties) Some of Lycoming & Snyder; parts of Union, Northumberland, and Upper Dauphin. Also growing in Schuylkill, Clinton, and Centre Top Experiences: Hospice/Home Health, Any critical/acute care- Med/Surg, Telemetry, ICU, Emergency Dept, Oncology, open to long-term care. Hourly: $29/hour Schedule: Part-time Sat & Sun 8am-8pm
