ASAP About the Position Pay Rate: $47.00 - $51.00/Hourly. Summary:
The Licensed Master Social Worker (LMSW) provides psychosocial
assessments, counseling, case management, and advocacy services to
individuals, families, and groups. The LMSW collaborates with
interdisciplinary teams to support patients’ emotional, social, and
environmental needs while promoting overall well-being and access
to resources. Responsibilities: Conduct psychosocial assessments
and develop individualized care plans. Provide counseling, crisis
intervention, and supportive services within scope of licensure.
Assist patients and families with coping strategies related to
illness, mental health, or life challenges. Coordinate care and
connect clients with community resources and support services.
Advocate for clients’ social, emotional, and environmental needs.
Requirements Requirements: 2 year experience Required, Skilled Home
Health Experience, BLS Certification, LMSW (Licensed Masters of
